Player Physical & Clearance
Be sure to follow the instructions to register your student athlete. Complete their medical info online. The School has a Physical night at Carlsbad High School where CHS Students can get physicals. If you choose to complete your physical at the physical night at CHS, pre-pay through Go Fan and print out (and have student take a picture on their phone) to show proof of payment.

Parents fill out a section and doctor fills out the rest (During Physical). Once complete, you will upload the completed document to the HOME CAMPUS site to submit for clearance.
